Medical treatment consent: I being parent/guardian of the said child understand that whilst every precaution will be taken to ensure the good welfare and protection of my child, Together Church's Staff, and it's volunteers are hereby released from any and all liability in the event of any accident or misfortune, damage or loss that may occur to the child & their property. In the case of an emergency, I hereby give permission to the first aid staff to ensure proper treatment for my child. I understand that every effort will be made to contact me before instituting such procedures. I agree to pay all such doctor, ambulance & hopsital fees incurred on behalf of my child. I have provided relevant health information about my child that the first aid staff need to know.
Involvement consent: I being the parent/guardian of the said child hereby give my consent that my son/daughter may participate in any activities the leadership chooses while at Together Church. I understand that everything taught and done at Together Church is age appropriate and suitable for every child in attendance.
Photography, video & social media consent: I being the parent/guardian of said child hereby give my consent for my son/daughter to be captured in both photographs and video at Together Church. Together Church reserves the right to use this material for promotional purposes. Together Church may be filmed and/or recorded by or on behalf of Together Church for production and/or resale. It is a condition of entry audience waives any claim he or she might have in relation to inclusion of their likeness in such film or recordings. I being the parent/guardian of said child hereby give permission for photos/videos/media of my said son/daughter to be placed on Together Church and its related departments on social media sites, including Facebook and Instagram. In circumstances where I, the parent/guardian, do not consent to this, I will provide and clearly communicate the reasoning to Together Youth so they are fully informed. Following this, Together Church will ensure this request that my child not be included in any photos/videos/media will be upheld.
Supervision policy: I being parent/guardian of the said child understand that Together Church will provide supervision inside the venue of Together Church during the event.
Communcation consent: I being the parent/guardian of the said child hereby give my consent for my son/daughter to be contacted by Together Church's Staff and its volunteers either by phone or electronic media for the purposes of discipleship. Together Church may utilise your provided information to contact you in regards to relevant upcoming events, programs and other news.
